CLINICAL SIGNIFICANCE
Cor (Cortisol) is the main glucocorticoid secreted by the adrenal glands. The main reasons for measuring cortisol are to diagnose disorders caused by cortisol overproduction due to Cushing's syndrome (CS) or adrenal steroid insufficiency due to Addy's disease, and to monitor treatment. Cortisol plays an important regulatory role in many important physiological processes, including energy metabolism, maintenance of electrolyte balance and blood pressure, immune regulation and stress response, cell proliferation, and cognitive function. The major part of cortisol is binding to plasma proteins as corticosteroid-binding globulin and albumin. The bioactive free portion only accounts for 2-5% of the total. Serum levels can rise during stress responses, mental illness, obesity, diabetes, alcohol tolerance and pregnancy, making diagnosis difficult in patients with Cushing's syndrome. Low cortisol levels are seen in patients with rare adrenal enzyme defects and in patients with chronic stress. Typically, cortisol production peaks later in the night and peaks in the early hours of the morning. Cortisol levels then gradually decrease throughout the day, reaching their lowest levels in the first half of the night. Therefore, the circadian rhythm of cortisol secretion and the influence of stress must be considered for sampling conditions of serum, plasma and saliva.
